Job Summary

$58,096 - $90,804 / year + Full-Time County Benefits 

Performs responsible work identifying and providing services to address the needs of children and their families through the provision of community-based services through the Child Health Initiative Program (Care Team).

The Community Health Specialist is a key member of the Care Team, working with caregivers and children who are facing challenges across various areas of their lives to support health education and healthcare navigation.

Responsibilities

• Provides ongoing assessment of health care needs for children and their families and develops health goals and objectives as identified through the assessment. This includes building trusting partnerships with families to understand their health, social, and economic needs.
• Provides case management services and navigation support to obtain and maintain health coverage, find appropriate healthcare providers, and coordinate medical care. This includes making referrals and communicating with health insurance systems and benefit administrators.
• Ensures children receive regular check-ups, vaccinations and other preventative services.
• Provides families with education on understanding care plans, medication regiments and follow-up appointments.
• Collaborates with healthcare providers, including school representatives, to ensure that care is well-coordinated, particularly for children with chronic illnesses, mental health or developmental needs, or other special health care needs. Supports providers with understanding the cultural context of the family’s needs.
• Provides families with health education to promote healthier living, including wellness activities and group learning.
• Collaborates with other Care Team staff members to facilitate consistent engagement with families in both home and community settings, ensuring regular communication and support.
• Documents accurately and timely within the case management software system, reports, assessments, family plans, progress notes, and other required forms.
• Performs other duties as assigned.

Qualifications:

• Associate’s degree in health, social services or related field required; bachelor's degree preferred.
• Certification as a Community Health Worker (CCHW), and/or Professional Public Health network membership is a plus. Public Health experience preferred.
• Experience working with children and families, individuals from vulnerable populations and home visiting.
• Experience in case management, including timely documentation of notes and data, assessments, decision-making and goal planning.
• Experience with navigating public sector programs, including Medicaid, WIC, SNAP, Social Security and/or other social services programs.
• Skill in use of computer software, especially Microsoft Office Suite. Experience with case management software a plus.
• Must possess, or obtain within 30 days of hire, a valid Virginia driver’s license and have an acceptable driving record based on James City County’s criteria.
• Knowledge of broad range of treatment modalities and disorders, to include mental and substance abuse, as well as knowledge of medications, indications, and contraindications
• Knowledge of principles and processes for providing customer service including setting and meeting quality standards for services, and evaluation of customer satisfaction.
• Skills in recording, interviewing, assessing, decision making, and health goal planning to include timely documentation of notes and data.
• Skill in use of computer software, especially Microsoft Office Suite; and case management software a plus; and oral and written communication
• Ability to analyze and use judgment in accomplishing diversified duties; think independently within the limits of policies, standards, and precedents.
